General Formatting Guidelines

• Phone numbers:  555-555-5555
• Dates:  mm/dd/yyyy
• No punctuation such as periods, commas or apostrophes; only hyphens are appropriate.
• For names with an apostrophe such as O’Malley or O’Brian – use the format OMalley or OBrian (no apostrophes and no space)
• Use an asterisk * in place of middle name if unknown
• Use only full names printed on birth certificate. If a student has a shortened first name or an unofficial English name, put this name in the Nickname field.
• A space counts as a character. Spaces do not mean blank.
